We report a case of Fournier's gangrene probably caused by a perianal ayscess. The patient was a 63-year-old man who was referred to our hospital on the 11th day after onset of the disease, complaining of perineal pain and scrotal swelling. Incision open drainage and debridement were performed to save his life. From our experience, attention should be focused on the following points for the treatment: (1) It is necessary to detect the disease as early as possible and to perform incision open drainage and debridement completely without hesitation. (2) At first, wide-spectrum antibiotics should be used, and they must be replaced by one having higher sensitivity based on the results of culture. Because bacterial alternation occurs due to administration of antibiotics, bacteriologic examination must be performed very often. (3) In many cases, there is a complication by shock or underlying disease such as diabetes, and treatment of the concurrent disease must be carried out together with management of systemic conditions.
