A 61-year-old man complanining of general fatigue and a loss in weight was admitted to our hospital for exploration of a large liver tumor which was detected by an abdominal CT scan at another hospital.<br> After admission an abdominal ultrasonogram and an abdominal CT scan revealed a liver tumor about 10cm in diameter in the lateral segment of the liver and swollen lymph nodes of No. 1 and No. 8. Abdominal angiography revealed obstruction of the left portal vein but did not disclose tumor stain and encasement of the hepatic artery. We suspected colangiocellular carcinoma and performed a left lobe resection of the liver. However, the tumor was diagnosed as poorly differentiated hepatocellular carcinoma with a sarcomatous change histopathologically.<br> We present here the case because hepatocellular carcinoma with a sarcomatous change is comparatively rare in Japan.
