Abstract

A 22-year-old woman who underwent an appendectomy with a diagnosis of acute appendicitis developed an intestinal fistula on the 8th postoperative day. Conservative therapy was unsuccessful and a laparotomy was performed on 101st day after the appendectomy. Based on intraoperative findings, the patient was diagnosed as having Crohn disease and underwent an ileotomy. The excised material revealed longitudinal ulcer and cobble stone appearance. Histopathological study also disclosed a granulomatous lesion extending to the full thickness, the definite diagnosis of Crohn disease being made. Repeated histologic study of the resected appendix revealed glanuloma without caseous necrosis, and it was thought that Crohn disease also existed in the appendix.<br> External intestinal fistula is a rare complication after appendectomy. If such fistula is intractable like in this case, allowances must be made for possible association of Crohn disease in the treatment.
