This paper presents a case of bilateral popliteal entrapment syndrome successfully treated by bilateral division of the medial head of gastrocnemius muscle in a 17-year-old man, who complained of intermittent claudication of the left leg and cold sensation of the bilateral foot regions.<br> Although popliteal entrapment syndrome is relatively rare, it is disease of note in differential diagnosis among juvenile peripheral diseases due to a decrease in patients with Buerger disease. In the treatment of the disease, operation becomes the primary therapy. In contrast to bypass operation for the obstructed popliteal artery which results in poor patency, division of muscle or fasciculus for the patent popliteal artery lead to a good patency. Therefore the disease must be suspected for juvenile patients manifesting ischemic symptoms of the lower extremities and every attempt should be made to diagnose and treat them early.
