連続心拍出量測定装置NICO^<TM>の心臓手術時の精度の検討 Clinical Evaluation of a Non-invasive Cardiac Output NICO Monitor during Cardiac Surgery

Novametrix社製NICO<sup>TM</sup>により測定した心拍出量(NCO)の精度を検討するため,肺疾患をもたない心臓血管外科予定手術患者7例を対象に,NCO,肺動脈カテーテルによる持続心拍出量モニター値(CCO)と冷水注入による測定値(TCO)を同時に記録した.TCOに対するNCOとCCOの相関係数は0.783(p<0.001)と0.909(p<0.001)であった.一次回帰式はNCO=0.72×TCO+1.16,CCO=0.88×TCO+0.30, Bland-Altman plotの結果は,NCOが-0.65±1.48l・min-1, CCOが-0.42±0.97l・min-1であり,NICO<sup>TM</sup>は心拍出量モニターとして有用と思われた.

The Non-invasive Cardiac Output (NICO) monitor (Novametrix medical Systems Inc., Wallingford, CT, U.S.A.) utilizes a minimally-invasive partial CO<sub>2</sub> rebreathing method to determine cardiac output by means of a differential form of the Fick equation. We evaluated the NICO monitor by comparing its output (NCO) with those obtained by bolus thermodilution (TCO) and continuous thermodilution (CCO) using a Baxter Vigilancemonitor (Baxter Healthcare Corporation, Irvine, CA). Seven patients undergoing cardiovascular surgery were included in this study. A total of 53 comparisons were made with TCO ranging from 3.4 to 13.2l•min-1. Regression analysis between NCO and TCO gives a correlation of r=0.783 with a slope of 0.72 and an offset of 1.16l•min-1. The linear regression coefficient between CCO and TCO was 0.909 with a slope of 0.88 and an offset of 0.30l•min-1. Bland-Altman analysis between NCO and TCO resulted in a bias of -0.65l•min-1 with precision of 1.48l•min-1. The bias and precision between CCO and TCO was -0.42±0.97l•min-1.
