To elucidate the cause of the severe state in acute viral hepatitis, the Th1/Th2 balance of peripheral CD4+ T cells was analyzed in acute viral hepatitis and in fulminant hepatitis. The study populations consisted of patients with acute viral hepatitis (AH n=17), the severe from of acute viral hepatitis (AHS n=9), fulminant hepatitis (FH n=9), and controls (n=35). The number of Th1 cells, Th2 cells, and the ratio of Th1/Th2 were measured using flow cytometry. IL-18 levels were measured using an ELISA kit and their correlation to Th1 cells, Th2 cells, and the ratio of Th1/Th2 was investigated. Th1 levels were significantly lower in FH than in AH (p<0.01). The values measured for FH tended to be lower than those measured for AH and AHS. The level of IL-18 was high in AH, AHS, and FH, but was not correlated with Th1/Th2.
