We present a rare case of rectal endometriosis with lymph node metastasis.<BR>A 50-year-old woman was admitted because of mucous, bloody stools and constipation. A barium enema and colonoscopy revealed a submucosal tumor and marked stenosis in the rectum. Pelvic CT showed bilateral ovarian cystic lesions and marked wall thickness of the rectum. Following the diagnosis of rectal endometriosis, a low anterior resection was performed. The resected specimen showed a submucosal tumor of the rectum, 4.5×2.5 cm in diameter, and bilateral chocolate cysts of the ovary. Histological observation revealed the tumor was endometriosis of the rectum with pararectal and intermediate lymph node metastasis.
