A 60-year-old man admitted for anorexia, general fatigue, and sore throat underwent abdominal computed tomography (CT) for abdominal distention from the transverse colon to the sigmoid colon and wall thickening at the ascending colon. Colonoscopy showed deep, punched-out ulcers in the ascending colon. Serum CEA and CA19-9 were high, indicating malignancy. Megacolon was associated, so right hemicolectomy was done based on a lack of perforation and localized inflammation of the right colon. Histopathological studies confirmed the diagnosis of simple ulcer or Behçet's disease. After operation, megacolon was improved and serum CEA and CA19-9 became normal. We diagnosed simple ulcer because Behçet's criteria were not made met. The postoperative course was uneventful. No recurrence was detected postoperatively.
