A 60-year-old man, diagnosed as having total situs inversus in his early childhood, was referred to the hospital for close examination because fecal occult blood test was positive. With abdominal CT scan, colonoscopy and barium enema study, ascending colon cancer with total situs inversus and intestinal malrotation was diagnosed. The patients was operated on. At laparotomy, the liver was located on the left side, while the stomach was located on the right side. In addition, the small intestine was located on the left side, and the colon was located on the right side. The Treitz ligament had not seen formed and the malrotation was demonstrated to be of non rotation type. A right hemi-colectomy with D3 lymphadenectmy was performed after the ileocolic, right ileal, and middle colic arteries which supply the ileocecum and ascending colon were defined. Histopathologically, the tumor was diagnosed as moderately differentiated adenocarcinoma, mp, ly0, v0, n0, and stage I. The postoperative course was uneventful and he was dischagerd from the hospital on postoperative day 18. Since Colon cancer with total situs inversus is rare, the case is reported here with a review of the literature.
