Gangrenous type ischemic colitis is a rare disease and the prognosis is poor in most cases. We encountered a case of gangrenous type ischemic colitis with the rapid onset that was associated with severe metabolic acidosis. A 64-year-old man was seen at the hospital because of abdominal pain. Emergency colonoscopy detected mucosal necrosis in the sigmoid colon, and an emergency surgery was performed on the same day. A small amount of ascites was observed (bacterial culture: negative), and necrotic changes were noted in the sigmoid colon. A sigmoidectomy and a temporary colostomy were performed (Hartmann's procedure).<br>On pathological examination, submucous hemorrhage in a wide area, mucosal necrosis, and many thrombi in small blood vessels were found in the sigmoid colon, and the diagnosis of gangrenous type ischemic colitis was made. No signs of recurrence have been observed as of seven months after the surgery. Although the mortality rate of this disease is high, this patient was successfully treated by surgery at the early stage. It is suggested that early diagnosis and surgical treatment are necessary for gangrenous type ischemic colitis.
