Japanese Guidelines for the Management of Hypertension (JSH 2000) published in June 2000 recommended that hypertensive diabetics lower their blood pressure to less than 130/85mmHg. Angiotensin converting enzyme (ACE) inhibitors are regarded as a choice for first-line drugs. Some ACE inhibitors improve insulin resistance, but little evidence is available on the effects of imidapril. o determine the effects of imidapril on insulin sensitivity, we studied 10 patients with hypertension and diabetes mellitus. Euglycemic clamp at insulin infusion of 40 mU/m2/min and 400 mU/m2/min was done before and after 12 weeks of treatment with 5mg of imidapril. Blood pressure fell from 154/85mmHg to 138/76 mmHg (p<0.05). In euglycemic clamp, the glucose infusion rate (GIR) increased from 4.6±2.6mg/kg/min to 5.5±2.1mg/kg/min (p<0.05) at 40 mU/m2/min of insulin, but no statistical difference was seen in GIR at 400mU/m2/min. These results suggest that the ACE inhibitor imidapril decreases blood pressure and improves insulin sensitivity.
