We evaluated the association between changes in periodontal status, general health, and serum antibody titers against periodontopathic bacteria over a 3-year period. Periodontal status was determined using the community periodontal index of treatment needs (CPITN). Parameters employed for analysis regarding systemic conditions were age, gender, body mass index, systolic and diastolic blood pressure, serum alkaline phosphatase, total cholesterol, high-density lipoprotein cholesterol, triglyceride level, white blood cell counts, platelet counts, smoking habits, and electrocardiographic ranks (ECG). Serum IgG antibody titers for Porphyromonas gigivalis (P. g) fimbriae, P. g whole cell antigen, and Actinobacillus actinomycetemcomitans (A. a) whole cell antigens of each subject were determined using enzyme-linked immunosorbent assay (ELISA). Subjects were classified into 3 groups based on changes in maximum CPITN code: decreased maximum CPI code, CPI (-) ; unchanged, CPI (0) ; and increased, CPI (+). At baseline, maximum CPITN codes in the CPI (+) group were significantly lower than those in the other 2 groups. The mean ECG score in the CPI (-) group was significantly higher than in the other 2 groups. We found that the number of teeth lost in the CPI (-) group was higher and mean serum total cholesterol (T-CHO) decreased significantly more than in the other 2 groups over the 3-year period. Mean changes in ECG scores in the CPI (-) group tended to be greater than in the other 2 groups. Further analysis showed that64% of the teeth lost in the CPI (-) group belonged to the CPITN = 4 category at baseline. No significant difference was seen in changes in the serum IgG antibody titers to P. g fimbriae, P. g whole cell antigen, and A. a whole cell antigen among groups over the 3-year period.<BR>We assumed that during the 3-year period, teeth with severe periodontitis at baseline were lost, improving periodontal condition, which in turn ended up with a significantly greater decrease in T-CHO.
