In 2297 cases of hollow organ cancer of the head and neck region treated in our hospital from Jan. 1990 to Dec. 1999, 11.2% had second primary cancers by Dec. 2000. Esophagus accounted for 29.1%, the head and neck area for 25.7%, stomach for 11.9%, and lung for 7.5%. Eighty-one cases had synchronous esophageal cancers. In 358 cases with hypopharyngeal cancers, 13.7% had synchronous esophageal cancers, of which 20 cases underwent total laryngectomy and esophagectomy at the same time. The cases which had radical mediastinum dissection had a higher possibility of necrosis of the end of the trachea than the cases which had transhiatal esophagectomy. To prevent this, we preserve the thyroid gland adhering to the trachea, or perform operations for the head and neck cancers and for the esophageal cancer separately.
