En bloc resection of the skull base with facial components and the reconstruction of the craniofacial defects were shown on a video and discussed. Important points for obtaining excellent surgical results are as follows: 1. En bloc resection of both primary and skull base invasion sites should be performed for complete removal of T4 maxillary cancer with skull base invasion. 2. Defects of dura, bony skull base, and facial components (both soft tissues and bony structures) after en bloc resection should be reconstructed safely and completely for obtaining good QOL. 3. Intracranial major complications should be prevented by cutting off all communication of the intracranial cavity from the outside world and by strict postoperative care. 4. Team work during the skull base surgery should be effectively perforned.
