We report a case of colon cancer with a colocolic fistula which developed 3.5 years after radiation therapy for lung cancer. A 59-year-old man was referred to the department because of left upper abdominal pain. Abdominal computed tomography and barium enema revealed colon cancer with a fistula between the transverse colon and descending colon.<br>Upon laparotomy a fist-sized tumor invading the gastric body and retroperitoneum was felt. Clinical stage IIIa was diagnosed, and left hemicolectomy and local resection of the stomach were performed. The pathological findings of the resected specimen showed moderately differentiated adenocarcinoma. The operation resulted in curability A. No hepatic metastasis, peritoneal dissemination, or lymph node involvement was seen. The postoperative course was uneventful. The patient has been followed in the clinic without signs of recurrence, as of 3 years 10 months after the operation. The incidence of the colon cancer with fistula formation between two parts of the colon is very low in Japan. Curative resection for colon cancer with fistula formation can lead to a better prognosis, in spite of its invasive character.
