若年者のＳ状結腸癌に合併した閉塞性大腸炎の１例

A 35-year-old male had an episode of constipation and left lower abdominal pain before admission. He had bloody stool after using a laxative. Colonoscopy revealed a type 2 tumor occupying two thirds of the circumference at the sigmoid colon. A colonoscope was inserted beyond the stenosed site and normal mucosa extending 20 cm from the oral side was seen, and longitudinal or circular ulcer was seen to the oral side of the transverse colon in splenic flexure of the colon. Left hemicolectomy was carried out under the diagnosis of obstructive colitis secondary to sigmoid colon cancer. The resected specimen showed a type 2 cancer (depth of carcinoma: se, histological pattern:poorly differentiated adenocarcinoma) measuring 5.4×3.2 cm in size. Longitudinal or circular ulcer (UL II) 10 cm in length and normal mucosa were found 16 cm from the obstruction. We reviewed 15 cases of obstructive colitis in young men in Japan.
