The incidence of benign bile duct strictures after hepatic resection is 3-5% and treatment is difficult and controversial. We report a case of benign bile duct strictures after extended right hepatectomy for colonic metastasis successfully treated with an expandable metallic stent. A 71-year-old woman who underwent low anterior resection, right hemicolectomy, and extended right hepatectomy for rectal cancer, colon cancer, and a huge liver metastasis underwent a total of 170 g of fluoropyrimidines, and her liver function tests often fluctuated in the 3 years following surgery. Liver function tests were elevated and not resolute. Abdominal US and CT showed dilation of the intrahepatic bile duct with no metastasis to the liver or hepatic hilar lymph nodes. Perctaneous transhepatic cholangiographic drainage showed the bile duct stricture. Balloon dilation through PTCD was repeated with temporary success. An expandable metallic stent finally achieved the desired result, and the patient remains disease-free and well.
