抄録

Objective: Mobile bearing total knee arthroplasty was developed to avoid wear and breakage of the polyethylene and loosening of the prosthesis. Some short and mid-term clinical results of LCS total knee arthroplasty are discussed.<BR>Methods: Since 1995, 1123 LCS (low contact stress) total knee arthroplasties have been done at Seirei Mikatahara Hospital on a one-surgeon basis. Pre- and postoperative Japan Orthopaedic Association knee scores (JOA score), range of motion, and complications were checked. Fluoroscopic studies of mobile and fixed bearing TKAs were examined.<BR>Results: Average JOA scores improved from 51 to 80 points postoperatively. The average postoperative flexion at 6 months was 120 degrees, and showed no significant difference from the 118 degrees flexion preoperatively. Extension improved from -7 degrees to -2 degrees. Postoperative flexion showed strong correlation with preoperative flexion. Mobile bearing TKA shows 15 degrees more flexion than fixed bearing TKAs on a one-surgeon basis. Fluoroscopic study showed significant rotation on flexion more than 90 degrees.<BR>Conclusion:<BR>1) Short and mid-term clinical results of 1123 LCS TKAs operated from 1995 to 2003 on a one-surgeon basis were examined.<BR>2) Postoperative improvements of JOA scores were satisfactory. The pre-and postoperative ROMs were the same.<BR>3) Fluoroscopy showed rotation on deep flexion more than 90 degrees.<BR>4) Soft tissue balance is important with mobile TKA.
