抄録

We report a case of malrotation of the intestine with midgut volvulus in an adult. A 21-year-old man was admitted to our hospital because of melena and upper abdominal pain. Abdominal computed tomography (CT) revealed that the superior mesenteric vein was located to the left of the superior mesenteric artery (SMV rotation sign) and that soft tissue had encircled the superior mesenteric artery (whirl-like pattern). A diagnosisd of malrotation of the intestine with midgut volvulus was made, and emergency surgery was performed. Intraoperatively, the small bowel was found to be located on the right of the abdominal cavity and the colon on the left. The ligament of Treitz was not formed, and the proximal portion of the jejunum was twisted clockwise 180° Removal of the volvulus and prophylactic appendectomy were performed.<br>Prompt diagnosis based on distinctive CT findings in this case enabled early operative treatment.
