A 67-year-old female had cough and bloody sputum for thirteen years from 1981 due to secondary bronchoectasia after pulmonary tuberculosis. Chest X-ray and chest CT demonstrated left destroyed lung with chronic empyema. Considering her quality of life, left pneumonectomy and omentopexy were performed simultaneously. The omental flap was suppleid by the right gastroepiploic artery and filled up the residual thoracic cavity completely. Therefore no additional procedure such as thoracoplasty or muscle transposition was performed. Postoperative course was uneventful and chest CT after surgery showed that sufficient omentum filled up the thoracic cavity with a low density level.<BR>The patient is alive 15 months after surgery without any evidence of recurrence or residual respiratory disease.
