頸動脈内膜剥離術における合併症の予防とハイリスク症例に対する対策  [in Japanese] Pitfalls and Their Recent Progress Countermeasure in Carotid Endarterectomy : Recent Progress  [in Japanese]

最近の研究により, 頸動脈内膜剥離術(carotid endarterectomy;CEA)は, 適応基準や手術, 周術期管理において, ある条件を満たせば脳梗塞の再発を予防する有効な治療法であることが明らかとなりつつある. しかし, 本手術の術中や術後に合併症が生じた場合は重篤な後遺症をきたしうることも事実である. 今回, われわれは合併症を予防するために実施している手術手技, 周術期管理, モニタリングなどについて, 特に, 75歳以上の高齢者, 高度の脳虚血や重度の全身合併症を有する例など, ハイリスク症例を中心に最近の経験を報告する. CEAの手術手技と周術期管理 2002年1月から2004年6月までの間に, 筆者が北海道大学病院, あるいは, その関連病院で実施あるいは参加したCEA30例31側を対象とした. 術前, 術中から術後にかけての周術期に, 合併症の発生を予防するためにわれわれが実施している注意点について述べる.

We review our recent experience of carotid endarterectomy (CEA) for 30 patients wit severe carotid artery stenosis. In this report, we emphasize the importance of critical management before, during, and after surgery to reduce or remove the incidence of perioperative complications Especially, we should carefully manage the patients who are at higher risk for hyperperfusion syndrome and those with complications of serious coronary artery disease. We also describe our surgical technique and results.
