A case of Reiter's syndrome with trismus

Abstract

Reiter's syndrome (RS) is a rare condition. Arthritis, urethritis and conjunctivitis are three major signs of this syndrome, and other clinical symptoms such as keratosis blennorrhagica and balanitis circinate also visible in patients with RS. Very few cases of RS complicated by temporomandibular joint symptoms have been reported. We recently eocountered a case of RS complicated by trismus. This case will be presented in this paper. The patient was a 33-year-old male. He was diagnosed as having RS, on the basis of pain in a number of his joints, a history of gonococcal urethritis, a history of iritis, balanitis circinate, keratosis blennorrhagica, positive HLA-B27 and negative rheumatoid factors. He began to receive drug therapy for RS. Although his systemic symptoms were alleviated, he continued to experience pain when he opened his mouth, and his mouth movement continued to be restricted. His superior articular cavity of the TMJ was washed by pumping, then injectioned with steroids. The pain was reduced and he was able to open his mouth wider after manipulation. However, there was still some restriction of mouth movement. He therefore received physical therapy to strengthen his mouth control, and his mouth extension improved. The results obtained in this case suggest that active local intervention can be useful when dealing with temporomandibular disorders associated with RS.
