Abstract
乳児集団健診の育児支援機能を探るため, 692人の母親に受診前後の母親としての自信と対児感情の変化, 属性などについての調査を行った(有効回答数429人)。その結果, 自信と対児感情の前後比較では, 健診後にすべての得点が有意に増加した。多重ロジスティック, モデルによる解析で, 自信得点の増加には, 不安の有無により, 尊重されている雰囲気が反対に影響する傾向を示し, 困ったことの有無により, 知識獲得の満足が反対に影響する傾向を示した。また, 接近感情得点増加には楽しめる雰囲気, 回避感情得点減少には自信得点の増加が関連していた。上記より, 健診場の雰囲気は不安のある人に影響を与えやすく, 健診後の母親としての自信の高まりには「尊重されていると感じる」場の雰囲気が重要であると考えられた。また, 対児接近感情の高まりには「児と楽しめると感じる」雰囲気が影響し, 対児回避感情の低下には「母としての自信の高まり」が関連していた。そして困ったことのある人に知識を与える時は, 自尊心への配慮が必要であると考えられた。また, 予備的知識の獲得が育児自信の向上に有効であり, 特に第1子の母親に対しての有効性が示唆された。
To evaluate the effectiveness of mass infant health examinations of growing children, we conducted an inquiry into the factors causing changes in self-esteem as well as affection toward their babies in 692 mothers before and after the mass infant health examination. Valid responses were collected from 429 women. The result of the inquiry showed that the scores indicating self-esteem and affection toward their babies were significantly increased among the responders after the mass infant health examination compared with those before the examination. A multiple logistic regression analysis was conducted to evaluate the factors that induced this change. The analysis demonstrated that the self-esteem score depended not only on the responders' feelings of being esteemed but also on the satisfaction involved with acquiring knowledge. This suggested that the pleasant atmosphere might have an effect on the increase in the sense of affinity toward the baby. Moreover, the increased self-esteem score might also be closely related to a decrease in the sense of alienation from the baby. Thus, our study showed that the atmosphere at the venue for the mass infant health examination was prone to affect mothers with feelings of anxiety ; in other words, the comfortable atmosphere contributed to a surge in self-esteem after attending the examination. Based on the above mentioned discussion, we concluded that when providing information regarding child-raising to the mothers (especially for their first child), their self-esteem had to be taken into consideration to help them improve their self-confidence.
