Abstract

Disc herniations at the cranio-vertebral junction causing cervical myelopathy were very rare. Only 5 case studies have been reported up to the present. We report a case of retro-odontoid large mass lesion and discuss the radiological finding, operation, pathogenesis and differential diagnosis with a review. 76 year old woman suffered from right upper limb weakness with hand clumsiness. An MRI demonstrated a mass lesion that compressed the spinal cord behind the odontoid process of C2. A mass was removed partially through an extremely far- lateral suboccipital approach, but additional posterior fixation was not performed. Histopathological examination could make a diagnosis of retro-odontoid disc herniation, because the specimen of the tumor consisted of fibrocartilage-tissue. 1 year later, an MRI showed that the mass had reduced in volume by itself, in spite of demonstrating a moderate instability, without symptoms.
