We report a case of chemical gastric bezoar development after gastrectomy by Gastrografin. A 70-year-old man with gastric cancer and rectal cancer was received distal gastrectomy (Billroth I reconstruction) and low anterior resection of rectum. 7 days after the operation, gastrography using Gastrografin was performed. 4 weeks after the gastrography, we confirmed a X ray with high absorption in the stomach through the abdominal roentgenogram as well as abdominal CT findings. We performed upper gastrointestinal endoscopy for diagnosing the intragastric alien substance. The egg-shaped blackish green bezoar with the size of 30×20mm was observed in the stomach and lithectomy were conducted using an endoscope. As a result of analyzing the crystalline substance, the presence of the amidotrizoic acid, the active ingredient of Gastrografin, was confirmed in 70% of the substance. This is the first case report of gastric bezoar formation that might involve Gastrografin.
