The final purpose of breast cancer screening is to reduce the rate of breast cancer. In order to do this, (1) a clear purpose must be distinctly defined, (2) there must be a proper distribution of resources, including participating staff, materials, funds, and information, (3) a good system must be established, and (4) successive planning, performance, evaluation and improvement (we have abbreviated this as the“Plan-Do-Check-Action” (PDCA) cycle) must be carried out. Furthermore, it is important to take high-quality mammograms and read them correctly, to establish a well planned screening system by dividing the whole prefecture into several appropriate screening districts, to adjust the screening method to individual districts, to control and evaluate the data obtained at a high level, and to compare them accurately with breast cancer registration data. <BR>Reviewing the present status of screening in Fukui Prefecture, the distribution of screening resourses appears generally reasonable, but the three technicians and two screening buses employed for the first screening were too few in view of the fact that as many as 31 medical doctors were availabe. Mass screening data were collected throughout all the prefectural districts by the Health Control Association, Fukui Prefectural Government, with a high level of quality and were well evaluated. The screening system using a bus was considered to have worked efficiently, matching the characteristics of the individual districts, and also from an economic standpoint. The standard of cancer registration was high, and our aims in the PDCA cycle appeared to have been properly met. In order to attain our final purpose, however, further efforts must be made to increase the participation rate, to plan a screening system that can cope with a 50% participation rate in view of the aging of the population, and also to encourage companies to participate more in breast cancer screening at the time of annual staff health checks.
