We evaluated 48 cases of lateral and anterior lower intrasphincter fistula that underwent sphincter-preserving operation (electrocauterizer sphincter-preserving operation) utilizing a needle-type electrocauterizer between April 2003 and March 2004. 1) Recurrence occurred in 4 cases (8.3%), while that of conventional sphincter-preserving operation (89 cases) was 7 cases (7.9%). 2) To determine the feasibility we compared the operative time between the electrocauterizer sphincter-preserving operation and conventional sphincter-preserving operation. The conventional operation took 16-31 minutes (mean : 21 minutes) ; the electrocauterizer operation took 7-12 minutes (mean : 10 minutes) ; the operative time in the electrocauterizer operation was significantly shorter than that of the conventional operation. 3) To determine the effect of the heat, we compared the cure period between the two groups. The conventional operation took 25-45 days (mean : 35 days) ; the electrocauterizer operation took 27-46 days (mean : 37 days).
