Congenital factor XII deficiency, which has no clinical bleeding tendency and is often detected by preoperative screening studies, is a rare entity. We report a case of the disease with cholelithiasis, in which laparoscopic cholecystectomy was performed safely without blood transfusion.<br> A 67-year-old woman was referred to our hospital for the purpose of laparoscopic cholecystectomy for gallstone. Preoperative screening studies revealed that the activated partial thomboplastin time prolonged to over 180 seconds. She had not experienced bleeding episodes in deliveries. Factor XII activity was found to be deficient, less than 1%. Activities of other coagulant factors (factor VIII, IX, X, XI, and XIII), the bleeding time and prothrombin time were within normal range. It has been reported that factor XII deficiency has a possibility of abnormal bleeding and thrombosis and that the activity should be normalized with transfusion of whole blood and/or plasma. The patient, however, had no history of abnormal bleeding and uneventful experiences with deliveries, so that we performed laparoscopic cholecystectomy without preoperative transfusion. The operation was not followed by any bleeding or thrombotic complications.
