抄録

A 69-year-old woman, who had been followed for idiopathic thrombocytopenic purpura (ITP) for about one year, was emergently admitted to the hospital because of abdominal pain and hemorrhagic shock due to anal bleeding. Lower gastrointestinal endoscopy offered a diagnosis of a malignant lymphoma of the ileum. Administration of a steroid to prepare surgery was unsuccessful in cotrolling ITP. Accordingly hand assisted laparoscopic surgery (HALS) was employed to perform splenectomy and right hemicolectomy simultaneously. The postoperative course was uneventful and the patient was discharged from the hospital on the 19^th hospital day. Thereafter the platelet count has been kept favorable and no recurrence of malignant lymphoma has occurred.<br> There have been few reports on malignant lymphoma developed in a patient who has been followed for ITP. In this case we employed HALS to perform splenectomy and right hemicolectomy simultaneously. We consider that HALS is beneficial even for high risk cases under steroid therapy in terms of the safety as well as low invasiveness.
