Abstract

A 47-year-old woman was seen at the hospital because of exacerbation of right back pain, which first appeared 10 years before entry. Abdominal CT scan showed a 40×80mm solid tumor with heterogeneous contents in the pancreas head and remarkable dilation of the main pancreatic duct. MRCP showed multiple cystic lesions in this pancreas head tumor. EUS showed a papillary growth mass in the main pancreatic duct. We diagnosed the case as intraductal papillary-mucinous tumor (IPMT) of the main pancreatic duct, and performed pylorus-ring-preserving-pancreaticoduodenectomy. Pathological diagnosis was invasive carcinoma derived from IPMT without surrounding tissue involvement and vascular invasion. Immunohistochemically, the tumor cells were negative for MUC2. The patient is under chemotherapy (Gemcitabine), and is still healthy without any signs of recurrence.
