Abstract

A 57-year-old man was transferred to the hospital after sustaining a blunt abdominal trauma in a car accident. He complained of lower abdominal pain with no visible skin abrasions. Abdominal CT scan demonstrated intraabdominal fluid accumulation and left kidney in pelvic cavity. One hour later, his abdominal pain increased, and an enhanced CT scan showed aggravation of fluid accumulation. Emergency laparotomy was performed two hours after the accident with a diagnosis of intraabdominal bleeding and hemorrhagic shock. At laparotomy, about 1100ml of bloody ascites, mesenteric injury 8cm in diameter and ischemic change of the small intestine 60cm proximal from the ileocecal valve were observed, so this ischemic portion was resected. The left kidney was confirmed to located in the pelvic cavity. Since an ectopic kidney generally develops with no symptoms, it often goes undetected. This is, to our knowledge, the first case of ectopic kidney with mesenteric injury accidentally detected by a blunt abdominal trauma to be reported in Japan.
