There is an anatomically weakened space in the lumbar area, but a hernia rarely occurs in the lumbar triangle. We report a case of right superior lumbar hernia repaired with a Composix Kugel patch^® using a laparoscopic procedure. A 72-year-old man was admitted for an elastic-soft mass at the right lumbar area. Computed tomography revealed herniated retroperitonial fat and offered a diagnosis of lumbar hernia. At operation a 2×2cm well-defined defect in the transversalis fascia, which forms the floor of the superior lumber triangle, was found under laparoscopy. Operative repair was done using a Composix Kugel Patch in the fascial defect. The postoperative course was uneventful, and he was discharged on postoperative day 4. This is the first report of repair with the Composix Kugel Patch with laparoscopic surgery in Japan. This method of tension-free herniorrhaphy is considered useful.
