EFFECTS AND PROBLEMS IN THE USE OF AN IMAGE-INTENSIFIER DIGITAL RADIOGRAPHY SYSTEM FOR GASTRIC CANCER SCREENING IN HUMAN DRY DOCK

Abstract

Recent advances in medical X ray devices have enabled the digitalization of gastric cancer screening. The image-intensifier digital radiography system (I. I. -DR) is steadily gaining ground as a tool for the X-ray examination of the stomach; however the advantage of using it has not been evaluated so far. The aim of the present study is to investigate the improvement of the accuracy of gastric cancer screening in human dry dock at our facility, through a comparison among three individual subject groups examined using different types of barium and X-ray devices, as follows: (A) 120% barium sol and a film-screen X-ray system (F. S.),(B) 180% high-density and low-viscosity barium and F. S., and (C) 200% barium and I. I. -DR. The number of examinees in three groups was (A) 11456,(B) 12140 and (C) 10890, respectively. At the second step of the screening, endoscopic investigation was performed on 82% of the examinees in group (A), 7.5% in groups (B) and 7.4% in groups (C) respectively. The rates of gastric cancer discovery were (A) 0.11%,(B) 0.18% and (C) 0.16%, showing a clear increase in groups (B) and (C). The use of high-density and low-viscosity barium raised the sensitivity of the X-ray screening of gastric cancer, I. I. -DR was able to increase the specificity of the screening in comparison to the F. S. system, because definite findings associated with gastric cancer, especially in the early stage, were detectable in 73.7% of cancer patients in group (B) and 85.7% in group (C). It is expected that the quality of gastric cancer screening will be improved by the use of the I. I. -DR system. In the near future, advanced systems of gastric cancer screening will be supported by various information technologies.
