A Case of Successful Thyroplasty for Sulcus Vocalis

Abstract

There have been many reports on treatment methods for sulcus vocalis, including temporal fascia grafting, silicone injection, fat injection, voice treatment, etc. It is difficult to select the optimal approach from among these treatment methods. We encountered a case in which thyroplasty Types I and IV were effective. The patient was a 72-year-old male with a chief complaint of hoarseness. An auditory impression indicated G (3) R (2) B (3) A (1) S (3), and MPT was 8 seconds. Strobolaryngoscopy revealed sulci on the free margins of the bilateral vocal cords. On the basis of these findings and observations, the patient was diagnosed as having sulcus vocalis. The left vocal cord with the deeper sulcus was treated by thyroplasty Type I, and the bilateral vocal cords were treated by thyroplasty Type IV. The hoarseness related to vocal onset improved postoperatively. These results suggest thyroplasty Type I to be useful as an alternative treatment of sulcus vocalis.
