Two cases of peripheral facial nerve palsy after orthognathic surgery

Abstract

Facial nerve neuropathy is a rare postoperative complication in orthognathic surgery. Once it has occurred, however, it is not only physically but also mentally a serious problem. We describe here two cases of unilateral peripheral facial nerve palsy following orthognathic surgery. Case 1: A 19-year-old male having mandibular prognathism with facial asymmetry underwent Le FortI osteotomy and bilateral sagittal splitting ramus osteotomy (SSRO). Seven days after the surgery, facial nerve palsy occurred in the left forehead, eyelid, cheek and lower lip areas. Pharmacotherapy alone was performed and the paralytic symptoms completely disappeared within 2 months. Case 2: A 20-year-old female having mandibular prognathism underwent bilateral SSRO. I mmediately after the operation, facial nerve palsy occurred in the right forehead, eyelid, cheek and lower lip areas. By pharmacotherapy in combination with physiotherapy, the symptoms improved moderately well 18 months after the operation.
