A case of imported hepatitis E: Identification of location of HEV infection by the molecular epidemiological approach

Abstract

A 30-year-old man visited Yunnan and Tibet Autonomous Region in China, Nepal and Thailand for four months since January, 2006. Soon after moved to Thailand, he noticed brown urine and jaundice and returned to Japan. Since he had IgM class antibodies to HEV and detectable hepatitis E virus (HEV) RNA of genotype 1 in serum, he was diagnosed with hepatitis E. He did not ingest unboiled water and uncooked dishes during the trip at all. However, he participated in the festival "holly" in Nepal and accidentally drank muddy water, which may have been associated with the acquisition of HEV infection. Upon comparison with the reported genotype 1 HEV isolates, the HE-JA06-1017 isolate recovered from this patient was found to be homologous to subgenotype 1a isolates and 100% identical to the 1a isolates that prevailed in Nepal in December, 2005, indicating that this patient contracted HEV infection while traveling in Nepal.<br>
