An extremely rare case of urinary bladder cancer metastasizing to the mandible is presented. A 64-yearold man was referred because of paresthesia of the lower lip and a well-defined, hard mass of the mandibular angle region. Oral examination showed a lump extending from the mandibular molar to the buccal region, covered with normal-appearing mucosa. Panoramic radiograpy and computed tomography demonstrated a lesion with bone destruction, measuring of 6×6×5cm in the posterior part of the mandible. The lesion was histologically diagnosed as undifferentiated carcinoma and irradiated. During radiotherapy, bladder cancer was detected, and grade 3 transitional cell carcinoma was diagnosed. Because the histologic features of the mandibular lesion were similar to those of bladder cancer, the former was diagnosed as metastasis from the latter. After the completion of radiotherapy with 56 Gy, which reduced the size of the mandibular lesion, the patient received three courses of systemic chemotherapy to treat the bladder cancer. However, severe myelopathy and electrolyte imbalance developed, and the patient died of pulmonary embolism.
