Clinical study of prostate cancer screening in maintenance hemodialysis patients treated at Kesennuma County Hospital

Abstract

Recently, patients with prostate cancer are increasing because of a propagation of prostate cancer examination. We report a clinical study of prostate cancer screening in maintenance hemodialysis patients treated at Kesennuma County Hospital.<br>Between July 2003 and April 2005, 94 male hemodialysis patients older than 50 years old were included. All patients were examined serum prostate specific antigen (PSA), and prostate biopsy was performed in patients who showed serum PSA more than 4.00ng/mL. Transrectal prostate biopsy was performed by ultrasound-guided 14-core biopsy. Age ranged from 50 to 84 old years (in average 65.8 years old), and PSA values were ranged from 0.15 to 63.81ng/mL (in average 2.94ng/mL). Ten patients showed PSA values of over 4.00ng/mL (7 patients ranged from 4.00 to 9.99ng/mL and 3 patients showed values of over 10.00ng/mL). We performed prostate biopsy in 8 patients after excluding 2 patients who were not indicated for biopsy. As a result, prostate cancer was detected in 3 patients. In this study, the prostate cancer detection rate was 3.2% (3/94 patients) and the biopsy positive rate was 37.5% (3/8 patients). These findings (3.2%) indicate a higher cancer detection rate than that among non-hemodialysis patients (0.89%). Because all 3 prostate cancer patients were under 70 years old and had clinically localized prostate cancer, they underwent radical prostatectomy.<br>The prostate cancer detection rate among maintenance hemodialysis patient was equal to or greater than that of non-hemodialysis patients. PSA screening test is important for early detection of prostate cancer.
