Abstract

Serum tumor makers for hepatocellular carcinoma (HCC), α-fetoprotein (AFP) and its fraction of lens culimaris agglutinin A-reactive alpha-protein (AFP L3), protein induced by vitamin K absence or antagonist-prothrombin (Factor II) (PIVKA-II), are widely used in clinical stage. They sometimes increase with non-cancerous condition, but it is extremely rare that they increase simultaneously in abundance. We reported a case of HCC, a 67-year-old man who experienced hepatectomy two times for HCC, with marked increase of tumor markers, AFP, L3 and PIVKA-II, and showed a mass lesion in preoperative examination. We misdiagnosed this case for recurrence of HCC and S4 liver resection was performed. Postoperative pathological examination revealed a xanthogranulomatous change but no HCC cells. The patient died about 11months after his third hepatectomy for postoperative complication of biliobronchial fistula. An autopsy showed no tumor recurrence in local or distant area.<br>
