Abstract

The case was a 48-year-old man who could not have dinner the night before because of nausea. Next morning he was found unconscious in the toilet. A blood study at admission revealed anemia and gastrointestinal endoscopy showed massive blood in his stomach, but the primary bleeding point was unclear. On the day following admission, a gastrointestinal endoscopy was performed again. It showed a submucosal tumor with ulcer at the ventricular angle in the stomach. We diagnosed it as the cause of the gastric bleeding. Laparoscopic-assisted partial gastrectomy was performed. This gastric tumor measured 3×2cm and had an ulcer in the center. The pathological diagnosis was aberrant pancreas with Langerhans islet, acinous cells and excretory duct (Heinrichtype I). Since an aberrant pancreas may, in rare cases, cause hemorrhagenic shock, a gastrointestinal problem should be treated with possible aberrant pancreatic involvement in mind.
