Abstract

A 75-year-old female was seen at the hospital for further examination of a lump in her right breast. Breast carcinoma was strongly suspected from mammogram and ultrasonogram. Gastric endoscopy revealed a type-2 ulcerative lesion, 5 cm in diameter, located in the posterior wall of the upper part of the stomach. But the biopsy specimen from the tumor in the stomach was not diagnosed. Modified radical mastectomy with axillary lymph node dissection, total gastrorectomy and splenectomy with D2 lymph node dissection were performed.<br>The tumor of stomach that spread under the mucosa was 12×10.5 cm in diameter, and another flat subserosal tumor, 4.5×4.0 cm in diameter, was found during operation. The breast tumor was 2.5×2.3 cm in diameter. Each histological diagnosis was the same, malignant lymphoma of diffuse and mixed type. Immunohistochemistry revealed T cell type that was positive for UCHL-1 and CD3. No metastases were noted histologically in axillary and stomach regional lymph node. Postoperatively, the patient received chemotherapy at another hospital. However, she experienced a recurrence in the chest local subcutaneous and liver, nine months after operation. The patient responded well to high-dose chemotherapy with self transplantation. The patient was doing well with no sign of recurrence for 2 years and 9 months. She died from sepsis after meningioma, 4 years and 3 months after the first operation.
