Abstract

A 57-year-old man complaining of right upper abdominal pain was admitted to our hospital. Colonoscopy revealed an unbordered invasive tumor without clear round wall nor ulcer formation, and its biopsy finding was well differentiated adenocarcinoma. Serum level of CEA was elevated to 3300 ng/ml, and abdominal computed tomography showed para-aortic lymph node swelling. An extended transverse colectomy with para-aortic lymph node dissection was performed and histological diagnosis was adenosquamous carcinoma with para-aortic lymph node metastasis (M1). 18 months after surgery he is still being followed by adjuvant chemotherapy and radiation. Primary adenosquamous carcinoma of the colon is rare, and its poor prognosis is reported. The efficacy of chemotherapy or radiation therapy has not been proved, so curative resection is the main treatment to improve prognosis.
