Abstract

A 63-year-old man with a 25-year history of anal fistula visited another hospital for radical operation in June 2004. A preoperative total colonoscopy revealed 3 tumors at the descending, sigmoid colon, and rectum. Biopsy showed adenocarcinoma. He was admitted to the hospital and underwent a left hemicolectomy and a partial resection of the rectum in August 2004. But three months after the operation, the perianal tumor enlarged gradually. Aspiration cytology of the tumor revealed adenocarcinoma. Local resection of the tumor was performed because the computed tomography showed no evidence of lymphnode metastasis and he declined to receive an abdominoperineal resection. The histopathologic findings were moderately differentiated adenocarcinoma. We diagnosed implantation metastasis of colon carcinoma to the anal fistula. Patients with anal fistula who have been operated on for colon cancer should be checked for anal fistula metastasis at follow up.
