Abstract
A 69-year-old woman was seen at the hospital because of a 3-week history of lower abdominal distension and vomiting. There was peritoneal irritation in the lower abdomen. Abdominal plain CT scan revealed free air in the abdomen. An emergency operation was performed with a diagnosis of diffuse peritonitis due to intestinal perforation. At surgery and about 3cm-sized small bowel tumor showing an extraluminal growth was identified on the contralateral side of mesentery of the small intestine about 1m distal to the ligament of Treitz. The infiltrating periphery of the tumor was adherent to the small intestinal wall where was about 5cm proximal from the tumor to form a hernia opening; and through the hernia opening the most part of anal side small intestine was incarcerated, forming an incarcerated internal hernia. The oral side small intestinal wall, which was adherent to the tumor's infiltrating periphery, was pulled and injured by the incarcerated intestine, revealing diffuse peritonitis. Thus the resection of the small intestine including the tumor and injured intestine was performed. Histopathological studies showed dense proliferation of bundle arrangement of spindle-shaped tumor cells. Immunohistochemically the tumor was positive for KIT, CD34, and α-smooth muscle actin and negative for S-100 protein. Gastrointestinal stromal tumor (GIST) of the small intestine was thus diagnosed. The patient has been disease free 2 years and 10 months after the operation.
