Abstract

We report a case of endometriosis of the appendix presented with melena.<br> A 43-year-old woman was seen at the hospital because of abdominal pain, pyrexia, diarrhea, and subsequent bloody stool which happened six days after her last menstruation started. Colonoscopy and barium enema demonstrated an extramural oppression around the appendiceal opening. Abdominal CT and magnetic resonance imaging scans showed 21×18mm enhanced tumor with smooth surface. We diagnosed it as an appendiceal tumor and performed ileocecal resection with lymph node dissection. Macroscopic findings revealed that the vermiform appendix had a white solid tumor at its root which showed hemosiderin deposition suggestive of hemorrhage. Microscopic findings showed endometrial stroma and glands from appendiceal subserosa to muscularis propria, which was compatible with the diagnosis of appendiceal endometriosis. The postoperative course was uneventful except minor wound infection, she was discharged from hospital on the 14th postoperative day.
