An 84-year-old woman was admitted to the hospital because of lower abdominal pain and high fever. On admission, there was slight tenderness in the lower abdomen. Hard nodules were palpable on a digital rectal examination and the white blood cell count, CRP, serum CEA and CA 19-9 levels were elevated. Abdominal CT scan showed an enhanced mass 5cm in diameter at the pancreas tail, a tumor 1cm in diameter in the liver, and ascites. From these findings, the patient was diagnosed as having pancreatic cancer with peritoneal dissemination and liver metastasis. Two days later, the lower abdominal tenderness became severe and Blumberg's sign and muscular defence in the lower abdomen developed. An emergency laparotomy was performed with a diagnosis of generalized peritonitis. We saw pancreatic cancer and small white nodules in the peritoneal cavity, and the metastatic lesion of the appendix which caused obstruction and perforation. Appendectomy and abdominal drainage were performed. The definite diagnosis of appendiceal matastasis of pancreatic cancer was made by histological examination. It is rare that appendiceal metastasis of malignant neoplasm causes secondary appendicitis. We report this case of perforated appendicitis caused by peritoneal dissemination of pancreatic cancer, together with a review of the literature. When we examine patients with advanced cancer presented with acute abdomen, secondary appendicitis due to metastatic cancer should always be considered as a probable diagnosis.
