Abstract

The patient was a 38-year-old man who had received medical treatment for hysterical psychosis craniocerebral-trauma epilepsy, and insomnia. On June 8, 2005, a Hartmann's operation was performed with a diagnosis of panperitonitis caused by idiopathic perforation of the rectum. The patient developed intractable high fever, fasciculation and rigidity of the extremities on the 4th postoperative day, and went into a coma with tremors on the 5th postoperative day. On the next day, unstable hemodynamics developed and rapidly progressed. He was taken to the ICU after neuroleptic malignant syndrome was suspected due to an increase of CPK to 1, 308IU/l. Dantrolene sodium was then prescribed. On the 8th postoperative day, a qualitative analysis of the myoglobinuria was positive, and the CPK level rose to 122, 370IU/l. The patient's general condition then gradually improved, and he was moved to the department of internal medicine on the 56 postoperative day.<br> We report a case of a patient with idiopathic perforation of the rectum, whose life was saved by prescribing dantrolene sodium to combat neuroleptic malignant syndrome, a disease with a death rate of about 10%.
