A 70-year-old man underwent surgery in another hospital with the diagnosis of acute cholecystitis due to gallstones. In the operative findings, the gallbladder strongly adhered to the transverse colon and could not be removed. Cholecystotomy was performed and only the gallstones were removed. He was admitted to our hospital for repeated symptoms such as fever and right hypochondralgia after discharge. Chilaiditis syndrome was diagnosed by abdominal X-ray examination showing intestinal gas in the right subphrenic space and a barium enema examination showing the hepatic flexure of the colon in the right subphrenic space. Abdominal computed tomography revealed hypoplasia of the median segment of the liver, thickening of the gallbladder wall and pigment stones contained. The gallbladder site was on the superior surface of the hypoplastic left hepatic lobe (suprahepatic gallbladder). Cholecystectomy was performed in this case. Hypoplasis of the hepatic lobe is a rare congenital anomaly that is sometimes associated with ectopy of the gallbladder and Chilaiditi's syndrome.
