A 79-year-old woman was seen at the hospital because of an episode of unconsciousness as well as hypoglycemia, and was admitted for close exploration. Abdominal CT scan showed a cystic tumor 30mm in longer diameter in the pancreas tail. Following endocrinological study and sampling from the hepatic vein after selective arterial infusion of calcium, a pancreatic insulinoma was diagnosed. Thus the patient was operated on. A possibility of multiple occurrence of the lesion could not be completely ruled out in the preoperative diagnosis, but intraoperative findings disclosed no lesions except the cystic tumor at the pancreas tail. The pancreas tail was removed which resulted in a favorable surgical outcome.<br> Pancreatic insulinoma is usually a solid tumor and rarely has a cystic change in it, so that we present this rare case here, together with a review of the literature. We also present some bibliographical comments on the evaluation criteria of the hepatic vein sampling after selective arterial infusion of calcium and operative procedures for pancreatic insulinomas.
