We experienced a very rare case of incisional hernia after laparoscopico-sacral resection of the rectum in a 79 year-old-man who complained of a bulge on his buttock. He had been aware of this subcutaneous bulge at an incision on the sacrum three months after operation. The bulge increased to fist size seven months after operation. Preoperative CT and MRI revealed that the content of the hernia sac was small intestine. A radical operation was performed by the mesh-plug method. The orifice of the sac adjoining the sacrum was 5cm in diameter and 7cm long. The sac was not opened and was repositioned into the pelvic space. Repair was done by the mesh-plug method. Moreover, the inferior third of the gluteus maximus muscle transposition flap with inferior gluteal artery was used to cover the orifice over the mesh. At nine months followup, there was no evidence of recurrent hernia.
